Navigating Care is replacing My Care Plus.

With Navigating Care, you can:

  • Access your health information 24/7
  • Receive personalized patient education
  • Be prepared for your doctor visits
  • Complete forms from home
  • Review lab and test results
  • Communicate with secure messaging
  • Connect with a community of others like you

For Current Patients Using My Care Plus

  • Check your inbox for an email invitation on August 20 from Navigating Care with the subject line “Join our Patient Portal.” If you don’t receive a link, check your spam or junk folders.
  • Follow the 3-step registration wizard to create your account
  • Bookmark navigatingcare.com/account/login for easy access

Continue to use My Care Plus until you receive an email to create a Navigating Care account.

For Current Patients Not Using My Care Plus and Interested in Enrolling in Navigating Care

  • Make sure we have your current email address on file if you haven't already done so
  • Check your inbox for an email invitation from Navigating Care with the subject line “Join our Patient Portal.” If you don’t receive a link, check your spam or junk folders.
  • Follow the 3-step registration wizard to create your account
  • Bookmark navigatingcare.com/account/login for easy access

My Care Plus Instructions

Access your account at: http://www.mycareplusonline.com

To send and receive messages through the Message Center:

  • Log in to My Care Plus 
  • Click the Message Center link

To send a message:

  • Click Compose Message from the menu
  • Select your doctor, enter your subject and message, and click Send

To view and reply to your received messages:

  • Click the Inbox link
  • Click anywhere on the message to open and read
  • Use the Reply Message box at the bottom to enter your message, and click Reply

  • Enrollment: If you have questions regarding enrollment in My Care Plus, please contact your NYOH practice.
  • Technical Issues: If you are experiencing technical issues, contact the My Care Plus site administrator Monday through Friday from 10:00 AM – 6:30 PM (Eastern Standard Time) at toll-free 1-855-887-6788.
